Her head doesn't look big, it is. Johansson has a big head. Face it. Most actors have big heads. It helps when you need to use it for your job. It also doesn't help matters when photographers take pictures head on (like this one), because it distorts the perspective and makes the upper body look larger than the lower body. Also, that dress is awful. You can tell she isn't super skinny because her arms still have quite a bit of that lovely, feminine *gasp* fat that prevents women from having those gross bricklayer forearm veins (I'm calling out YOU Madonna!). This is a fashion thing that didn't work for her, not a skinny thing. So take it easy on the poor girl!
